Accomplished Activities
· Participated in a local hour-long radio show, "Perspectives," to discuss issues
surrounding death and dying.
· Coordinated informational articles about the Moyers Series to run in a local
newspaper, agency newsletters, and a local hospice newsletter.
· Created and distributed bulletin inserts to local members of the faith community.
Inserts included details about the Moyers Series as well as contact information on
end-of-life issues (such as advance directives, general hospice information, etc.).
Upcoming Activities
· Partnering with other agencies such as, Big Bend CARES, The Senior Center,
Dept. of Elder Affairs, and the local art & theatre community. This partnership
will aid in workshops, forums, gallery events, and seminars.

Accomplished Activities
· Formed a speaker's bureau to discuss end-of-life issues including advance care
planning, the impact of grief in the workplace, and palliative care.
· Participated in a statewide "Pop & Watch" project.
Upcoming Activities
· Working on a twelve part series that kicked off the last Wednesday of September.
The series is collaboration with WLRN in an effort to keep end-of-life topics in
the mainstream. The "WLRN Series" covers twelve topics including: Facing Death;
Medical Treatment at the End-of-Life; Hospice; Legal Issues; Children and
Bereavement; Medical Ethics in End-of-Life Situations; Economic and Financial
Matters; Mind and Body; Pain Management; Special Therapies; AIDS; Diverse
Cultural Approaches to Funerals and Memorials. This Series will have a web site
that is currently being developed.
A live panel discussion will air after the last episode.
· An "End-of-Life" conference is being offered for the future and will target the
general public. This event will collaborate Dr. Ken Goodman's ethics conference,
which will target the medical community. The conference is scheduled
for March 2001.

Accomplished Activities
· Worked with a local community college to provide CEU/CMEs for viewing the
Moyers Series, this was highly publicized in the professional community.
Local PBS was also key promoter.
· Created and distributed brochures and flyers promoting the Moyers Series, as
well as providing general information about end-of-life alternatives and different
resources available in the community. Distribution included physician offices,
senior centers, and churches.
Upcoming Activities
· Plans to hold educational sessions every other month in various areas throughout
the three-county-area. Barnes & Nobles has offered to hold the seminars.
Citrus County Coalition

Accomplished Activities
· Met with church youth group leaders and youth group parents to discuss
end-of-life issues. Distributed an "agreement to watch as a family" for the
parents and teens to sign as well as the "On Our Own Terms" (The Moyers Series)
information pamphlet. Other local churches also got involved by inserting
information on the local Day of Decision in their Bulletins.
· Spoke to a Lions Club about advance directives.
· Purchased the Moyers Series to make available for community viewing.
Upcoming Activities
· Planning to compile, and publish, a resource guide for the Citrus County, which
would include all agencies, organizations, and groups that provide end-of-life care.
· An advance directives tool kit, which will include instructions, sample living will,
and health care surrogate forms, is a second goal.
Dade/Monroe Counties Coalition

Accomplished Activities
· Created paycheck stuffers with information about the Moyers Series with the help
of local PBS station. 150,000 were distributed to employees in corporations
throughout Dade county through the Employee Assistance Program
· Coordinated an Advance Directives Seminars for Health Care Professionals, one
location being Parkway Medical Center, with participants from Dade, Monroe,
and Broward counties, Karen Goldsmith, Esq. was the speaker. Information packets
with Do Not Resuscitate information, details about the Florida Partnership, and
other useful end-of-life information were distributed.
Upcoming Activities
· Working on a Facilitator Training Program. This program will be for
clergy and congregational lay leaders in the community and will focus on providing
a model for facilitating group discussions regarding end-of-life care. Attendees
will also be provided a packet of end-of-life resource materials. Approximately
100 clergy members are expected to attend the event.
· Planning to participate on another live radio show in November with Jane Hendricks,
Esq., regarding Advance Directives.
· Planning an End-of-life Resource Guide to be distributed throughout the community.
Northeast Florida Coalition

Accomplished Activities
· Mailed flyers with details about the Moyers Series to the St. Vincent 55+ Club
with 14,000 members
· Distributed over 5,000 "Isn't it Time We Talk About It?" buttons to volunteers,
staff, and the public.
· Community organizations got involved in each county and served alongside the
coalition in an "outreach blitz" effort. The outreach efforts focused on
providing, on-going information, education, updates, as well as recruiting
coalition members.
· Visited many culturally diverse churches and organizations in the community,
as well as the Cathedral Foundation, which provides housing to 600 senior
residents. Distribution of end-of-life literature and six discussion groups
have been completed with one session on the Five Wishes Living Will.
Upcoming Activities
· Coalition is working with the faith community to coordinate efforts for the
"Compassion Sabbath" event.
· Focus in the coming months include increased education and awareness of
end-of-life issues within culturally diverse communities, working with large
corporations to provide seminars on "Grief in the Workplace," and
Caregiver seminars.
· Completed a community end-of-life needs assessment that was distributed
throughout the community prior to the Moyers Series. The assessments are
currently being returned and the results are being tabulated by the coalition.
This information is being used to develop projects that focus on the needs of
the community.
· The Mayor will be on hand to kick-off "Day of Decision". He and his wife will
also be signing Advance Directives at a lunch provided at City Hall.
Hillsborough County Coalition

Accomplished Activities
· Printed & Distributed 20,000 bookmarks to 17 Hillsborough County libraries and
10 bookstores. The remaining bookmarks were distributed to many volunteers,
medical professionals, and the general community through a large health expo.
· Coordinated participation of a counselor, resource coordinator, clinical team
leader and admissions manager in an "Isn't It Time We Talk About It" call-in
show that aired prior to the Moyers Series.
· Coordinated the Day of Decision for September 6th securing Hillsborough County
Legislative Coalition Chairman and Vice-Chairman as co-hosts. Mailed thirty-five
invitations to current legislators and candidates. Received media coverage from
the Tampa Tribune regarding the Day of Decision Legislative Ride-A-Longs.

Accomplished Activities
· Started a pain consortium with local nursing homes. This consortium is working
with over twenty directors and nursing home staff on pain management issues such
as changing their documentation tool, etc.
· Developed activities to support the Moyers Series including: end-of-life book
displays at three area bookstores; flyers; bulletin board displays; air time on
the Hospital information channel at Indian River Memorial Hospital; television
clip on the Public Health Department Channel and the County Government Channel;
interview on small radio and television stations that serve predominantly African
American and Haitian communities; appearance on a local features program on a
station that serves mainly the Indian River County Area; feature article on the
coalition makeup with the focus and activities in the local paper; flyers at
area nursing homes, Assisted Living Facilities, adult daycare, and senior centers.
Upcoming Activities
· Planning a CME program on end-of-life/palliative care for 2001.
· Researching the possibility of: an end-of-life guide/resource book that could
be sponsored by community agencies or businesses; a Mother's Day program to
focus on the relationship of mothers and daughters on end-of-life issues; and
participating in research in social work education and end-of-life issues with
the UNC Chapel Hill.
· Hope to get "Project Decide" going, which would hold an informational seminar
for sons and daughters near or around mothers/fathers day.
Lake/Sumter Counties Coalition

Accomplished Activities
· Worked with other coalitions and a local cable provider to purchase a
thirty-second television spot with 251 spot placements. The spot aired on
six channels including CNN, Lifetime, and the Discovery Channel.
· Worked with Winn-Dixie Supermarkets to place shopping bag stuffers announcing
the Moyers Series in customer's bags. 50,000 stuffers were distributed via six
Winn-Dixie stores over the five days immediately preceding the first Moyers
broadcast.
· Obtained commitments from all three local daily newspapers to run advance
feature stories about end-of-life care. In addition, a two-page feature press
release using a variety of end-of-life information was written and faxed to the
two county area's eight weekly newspapers.

· Coordinated a Lecture Series including two lectures regarding the Art in
Healing. Guest speakers were from the Art Therapy Department at Shands
Hospital. Medical/Health professionals along with members of the community
attended these interactive lectures
· Coordinated two 1/2-hour shows with the local PBS station that focused on
services available in the community regarding end-of-life care.

· Worked with local PBS station to produce thirty-minute segments after
each Moyers broadcast involving local clergy, physicians, and hospice
volunteers.
· Provided information for a two page story in a local newspaper entitled
"Now I know I'm going to die" featuring the local hospice. The article
also included an insert about living wills and advance directives. To
date sixty calls for copies have been received and more requests are coming
daily. Other features, such as "Learning How to Live With Dying"
"Dying Wishes" and "Matters of Life and Death" followed.

Dr. William Allen of the University of Florida.
Pinellas County Coalition

· Announced the Day of Decision (proclaimed by Governor Bush) in local
newspapers with a mail-in coupon for advance directive packets. Over
1,200 advance directive packets were requested and distributed.
· Conducted follow-up visits to the 38 congregations who are partners in
The Hospice's Caregivers United program encouraging them to post
"Isn't It Time We Talk About It" advance directive posters and distribute
book marks promoting the Series.
· Distributed "Grief in the Workplace" brochures to the top 117 employers in
Pinellas County.

Mid-Florida: Polk/Hardee/Highlands Counties Coalition

· Mailed "A Call to Conversation" Sabbath kits with a letter, sample bulletin
insert, and Personal Values Inventory to area churches. The bulletin insert
was distributed throughout the community.
· Coordinated a fall seminar to discuss end-of-life issues. This seminar had a
panel of end-of-life experts and the audience was encouraged to ask any
questions relating to end-of-life.
· Provided Grief Support to Southern Living (ALF) staff.
Upcoming Activities
· Based on a recent study conducted in the community, a need for education of
advance directives was discovered. The coalition is developing a plan to
clarify and educate the community as well as exploring ways to make advance
directives more accessible and easier to understand.
· Planning a five-part bereavement series to be conducted by a chaplain of
Florida Hospital.
· Considering adoption of the advance directive forms and educational material
developed by "Project Grace."
Sarasota/Manatee Counties Coalition

Accomplished Activities
· Conducted town hall meetings for seniors where flyers to promote the
Moyers Series were distributed. The result of this meeting guided future
activities and programs.
· Participated in the statewide "Pop & Watch" project.
· Printed and distributed 50,000 "Isn't It Time We Talk About It" bookmarks
to libraries and bookstores the four county area.
· Contacted representatives of local schools to discuss possibilities of
conducting interviews to discuss end-of-life issues as part of life skills.
· Scheduled a town hall meeting following the Moyers broadcast at a local Senior
Center (Sr. Friendship Centers, Inc.) in Sarasota. Surveys were distributed
and group discussions were lead by an Elder Care Attorney and a Geriatrician.
Upcoming Activities
· A faith-based workshop is being planned that focuses on end-of-life issues.
This workshop will be followed by the Conversation Sabbath weekend.
· Some plans for the future include: development of a Speaker's Bureau and
Resource Directory, on going town hall meetings, additional seminars,
development of a coloring book for younger children in homes where someone
is dying.
· Pursuit of State & National legislators to participate in "shadow program."
· The first Sunday in November is being targeted as the day with plans to
encourage congregations to participate in end-of-life activities. Letters,
bulletin inserts, and sermon topics will be distributed.
Southwest Florida Coalition

· Provided informational flyers to local churches, area counseling centers,
and other local community centers. (Five days before the Series, thirty
advance directives were requested and distributed at a local Wal-Mart in
less than two hours).
· Established a toll free number for viewers to use following the Moyers
broadcasts, to recruitment of members, and to develop of ongoing events.
· Held three community discussions at Barnes and Noble in Ft. Myers and
Naples with more than 150 attendees.
Upcoming Activities
· Planned an all day "Train the Trainer" workshop for November 2nd at FGCU.
Workshop participants will be professional from organizations such as EMS,
the fire department, funeral homes, and physicians. These professionals
will then train their co-workers on end-of-life issues. CEUs and CMEs will
be available to participants.
Volusia/Flagler Counties Coalition

Accomplished Activities
· Placed flyers among employee's paychecks prior to the Moyers Series.
In addition to these employees receiving information, all Halifax employees
received an email about the September 6th Day of Decision, and calls were
made to local legislators to inform them of the importance of this event.
· Met with physicians in an effort to develop a physician public forum. This
forum will enable direct access to doctors for conversation on end-of-life care.
· Worked with the local United Way chapter to use their "First Call For Help"
line to take calls during and after the Moyers Series.
Upcoming Activities
· Planning an end-of-life photography project. Coalition has recruited three
photographers with many spin-offs for the idea.
· Working with local PBS station to produce a one-hour program focusing on
end-of-life issues from the consumer's point of view. This program titled
"Joan's Story" is centered around one of the coalition's most involved members.
Recently being diagnosed with cancer. Now on the other side she has agreed
to tell her story.
· Established a system for tracking the calls that will come in after the
Moyers Series. This information will be used as a base from which to develop
a community needs assessment.

· Worked with local PBS station to conduct pre-Moyers and post-Moyers
educational activities focusing on end-of-life issues and advance care
planning were conducted.
· Designed a kit entitled "Final Wishes" - Planning for the End-of-Life.
The kit includes information on estate planning, organ donation, cremation,
hospice care, funeral home directories, extended living facility directories,
hospital directories, and nursing home directories. These kits are being
offered free of charge to the community. (To date 500 kits have been
requested and distributed).
